Student Immunization Requirements
State law requires that all children attending a Wisconsin school be immunized against diphtheria, tetanus, pertussis, polio, measles, rubella, mumps and varicella.
2013-14 minimum immunization requirements for attendance at Wisconsin public, private, and parochial schools are as follows: Grades K-12 -- (4) DTP/DTaP/DT, (4) Polio, (2) Measles, (2) Mumps, (2) Rubella, (3) Hepatitis B, (2) Varicella.
Students attending 4K require (4) DTP/DTaP/DT, (3) Polio, (1) Measles, (1) Mumps, (1) Rubella, (3) Hepatitis B and (1) Varicella.
Students in grade 6-12 must also have (1) Tdap vaccine.
All students must have a varicella vaccine (for chicken pox) or have reported having had the disease. This year students entering kindergarten, grades K-12 must have had (2) varicella immunizations.

Just Kids Dental Program
Just Kids Dental is a school-based dental health program focused on providing children in Kindergarten through 8th grade with oral health education and preventive dental care, including cleanings, fluoride treatments and sealants. These services are at no charge to either the school district or eligible families and administered by a local hygienist...right within Superior schools!
Current participating Superior schools:
Four Corners Elementary
Great Lakes Elementary
Lake Superior Elementary
Northern Lights Elementary
Superior Middle School
Children are eligible if enrolled on Wisconsin Medicaid (such as Badgercare and Forward Health) or families are uninsured and income eligible within free/reduced lunch guidelines. Consent forms can be printed from the file folder under Just Kids Dental below or are available from the school’s nurse or secretary. Please note: a signed parental consent must be returned to the school nurse before a child may receive dental services. For more information check them out at www.justkidsdentalinc.org
